I agree that by typing my initials and name it is an electronic signature and is therefore legally binding. If the person participating in the Senior Trip is an individual that is over 18 years of age, they assume the below responsibilities and accept the below terms of agreement for themselves.

As parent(s)/guardian(s) of the minor child named below, I hereby consent to the said person attending and participating in any and all approved activities of the event entitled “Senior Trip”. This event will commence on May 26th, 2020 and end around 2:00 am on May 29th, 2020, barring any unforeseen circumstances. If any injury to my child occurs, I expect to be notified. In the event that my child needs medical treatment, I authorize the said chaperones to obtain medical care as is necessary.

If my child is uncooperative, belligerent, or disrespectful to another person, student, or chaperone, unruly, difficult to control, unresponsive to chaperone directive, is found in the presence of, or in the possession of alcohol or illegal drugs, is found in a hotel room alone with a member of the opposite sex after midnight, or is engaged in any behavior that may be deemed inappropriate by the chaperones, I authorize a chaperone to contact me immediately and I agree to pay all expenses which occur in sending my child home. I understand that a chaperone will stay with my child for a reasonable amount of time necessary to make arrangements to be sent home. E-cigarettes and vaping will not be allowed.

I hereby waive, release, and discharge from liability the Senior Trip committee, volunteers, chaperones, sponsors, Michelle Cowles and Too Much Fun, LLC, from any and all liabilities and/or claims that may arise from my child's participation in the Senior Trip.

I also assume full responsibility and liability for any damages caused by my child and for any property that is lost, stolen, or damaged, and for any injury, accidental or intentional to my child while participating in the Senior Trip. My agreements herein are in consideration for my child's participation in the Senior Trip. My child's signature below affirms that I have discussed the terms and conditions of this Liability Waiver with my child and that he/she has agreed to abide by and be bound by its terms and conditions.
